Additional Information

24-Hour Coverage: We have a physician on call 24 hours a day, 7 days a week. The answering service will forward your message to our trained registered nurses.

Other Services: We have a full-time nurse to give you phone advice should you have questions about the need for an appointment or another health matter. We also offer such services as evaluations for attention deficit disorder and NYS vaccines and children.

Cardiology

Cardiology is a subspecialty of Internal Medicine and is the study and treatment of diseases of the heart and blood vessels. These diseases include hardening of the arteries (atherosclerosis), heart attacks, palpitations and other diseases of the electrical system of the heart, congestive heart failure and a wide variety of other related problems.

Finger Lakes Medical Associates is partnering with Rochester General Hospital, the region’s only Top 100 Heart Hospital, to provide cardiac care services for the Finger Lakes communities. 6 RGH board-certified cardiologists have joined with us, offering a wide range of tests and treatments. These include cardiac catheterizations performed at Rochester General Hospital, sophisticated and specialized stress testing in Geneva, Intensive Care Medicine, and state of the art echocardiography.

Preventative medicine is one of our major priorities and is integrated with a Cardiac Rehabilitation program. Cardiology patients are seen in our Geneva office.

Internal Medicine

Adult Medicine is the medical specialty devoted to the physical well-being of adults (after pediatric care to geriatric). It encompasses a wide range of services from preventive care such as annual exams to the diagnosis of acute and chronic conditions to the non-surgical treatment of disease and injury.

By selecting an internist for your health care, you have chosen a physician with the highest level of medical training and experience in the care and treatment of adults. Adult Medicine patients can be seen in either our Geneva or Penn Yan locations.

Nephrology

Nephrology is the study and treatment of kidney disease. Marc Sadovnic, MD, is our practice specialist in kidney disease and hypertension and also sees patients at the Geneva General Hospital Dialysis Unit. He also has an established Internal Medicine practice and is pleased to see patients with or without kidney disease.

Obstetrics and Gynecology

Obstetrics is the medical specialty devoted to the care and treatment of women throughout pregnancy, childbirth, and the postpartum (after the birth of the baby) period.

Gynecology is the branch of medicine that has to do with the physical well being of women. It encompasses a wide range of services from preventive care such as annual exams and Pap smears to the diagnosis and treatment of acute and chronic conditions. By selecting an obstetrician/gynecologist for your healthcare, you have chosen a physician with the highest level of medical training and experience in the care and treatment of women. Our obstetricians/gynecologists are certified by the American College of Obstetrics and Gynecology.

Pediatrics and Development-Behavioral Pediatrics

Pediatrics is the medical specialty devoted to the physical, social, and emotional well being of children from birth to young adulthood. It encompasses a range of services from preventive healthcare such as well-child physicals and immunizations to the diagnosis and management of acute and chronic diseases. By selecting a pediatrician for your child’s heath care, you have chosen a physician with the highest level of medical training and expertise in the care and treatment of children. Our pediatricians are certified by the American Board of Pediatrics and are Fellows of the American Academy of Pediatrics.

Development-Behavioral Pediatrics is the specialty of one of our pediatricians, Jane McCaffrey, MD, who is certified by the American Board of Pediatrics in Developmental-Behavioral Pediatrics. Dr. McCaffrey was one of 390 physicians nationwide who were qualified to sit for the first certification exam in the this new sub-specialty, which draws on elements of neurology, child psychology, and psychiatry as well as general pediatrics to help children with a variety of problems including Attention Deficit Hyperactivity Disorder, learning disabilities, developmental delay, cerebral palsy, mood disorders, and Tourette’s syndrome. Dr. McCaffrey sees children with developmental-behavioral concerns who are referred by their primary care physicians.

Urology

Urology is the branch of medicine which concerns itself with the urinary tract in both men and women, and with the genital organs in the male. Richard Kasulke, MD performs in-hospital and in-office procedures including vasectomies and treatment for urinary incontinence.

Financial and General Policies

Thank you for selecting Finger Lakes Medical Associates, LLP for your health care needs. These financial and office policies are meant to help you prepare for your visit and help us serve you better. We will work hard to make your visit with us pleasant and comfortable.

Insurance Cards
If you have insurance, your insurance membership card must be presented at every visit. Please make sure you bring it with you, as the receptionist will ask to see it.

Patient Information
Be prepared to give us the most current information regarding the patient and/or guarantor:

  • Patient name or Guarantor
  • Address
  • Telephone number
  • Insurance company
  • Insurance address
  • Insurance policy number

Patient Responsibility
Parents and/or their guardian are personally responsible for all charges and/or balances for services rendered. If we participate with your insurance plan, we will bill it. If we do not participate with your insurance, payment is expected at the time of service.

Co-pays and Deductibles
All co-pays and deductibles are expected at the time of your visit. The cashier in each department will be expecting to collect these amounts as you check out after your visit with the physician. Failure to pay your co-pay at the time of service will result in a $10 administrative service charge.

For non-HMO plans, the co-pay expected at the time of service is 20% of your charges for that visit. Over payments will be refunded.

Worker’s Compensation Claims
Please let us know at the time you check-in if you are here for a Worker’s Compensation claim, No Fault Insurance claim, or Disability claim. We will need all necessary billing information including your employer information.

Notice of Past Due Accounts
Payments on all outstanding balances are due in full within 30 days of receiving your monthly bill UNLESS a payment plan has been arranged with our billing staff.

If we have not heard from you after 60 days, we will automatically refer your unpaid account to collections for further action. Referrals to the collections agency could adversely affect your credit.

Bounced Checks
All returned checks will be subject to a $30 fee, which will appear on your statement. We may also choose not to accept checks from you for future payments.

Credit Balances
Credit refunds are processed and mailed out on a monthly basis. If you have a credit balance with us, you will receive a check from us after all outstanding charges have been paid.

Charge slips/Receipts
After each office visit and/or payment, we will give you 1 copy of our charge slip for your visit or a receipt for payment. Please retain all copies of charge slips and/or receipts for your future reference. Yearend requests for additional copies will result in a $10 charge.

Disability Forms
Please complete the personal data section on all disability forms and signs the forms before submitting forms to the physician’s office staff for processing.

Request for Release of Medical Records
All requests for copies of medical records generated by Finger Lakes Medical Associates LLP during the course of your relationship must be in writing and signed by the patient or guardian. Please allow 2 weeks for the processing of records. There is a charge for copying medical records.
